OUR CARE
Victoria Care Centre provides specialist, around-the-clock care for young adults with long-term neurological conditions, such as brain injury, stroke and Disorders of Consciousness such as vegetative state and minimally conscious state and people with tracheostomy.
We recognise that it can be challenging to move into a nursing home. Our aim is not just to care for your condition but to care for you as a person. We will work with you to ensure you get the support you need and to give you the best possible quality of life.
When you first move in, our staff will support you to help you settle in and adapt to your new home.
We offer a range of social activities to help you meet people and feel at ease. We also provide a wide range of therapeutic and leisure activities to support your recovery, wherever possible, and to help you to manage your symptoms and live a full and active life.
MULTIDISCIPLINARY APPROACH
Your care plan will be created by a multidisciplinary team, each of whom will bring their individual skills, perspectives and experience to the management and treatment of your neurological condition. You and your family will play an active role in the creation of the plan, which will be regularly reviewed, monitored and updated.

REFERRALS
Using the Decision Support Tool, NHS Continuing Healthcare (CHC) teams might advise you to contact us. On receipt of this referral, we will make pre-admission arrangements and send you a detailed report outlining your long-term nursing and therapy care proposals, plus our weekly tariff. We will arrange for you and your family to visit our service and meet our team. `Once all of the arrangements are in place, we will arrange for your safe admission, by liaising with you and your family, plus CHC managers, and frontline professionals.
We work with case managers, case management companies, legal teams and insurers, both during any litigation process and after settlement. We facilitate effective communication between all parties involved in the litigation process. The aim is to ensure that you receive the nursing care, support and therapy you need within an agreed budget.
